PLEASE READ THE FOLLOWING 3 POINTS CAREFULLY

  1. If you need to cancel, complete and submit the form below at least 5 days prior to your next scheduled payment. This will serve as your written cancellation notice as required by your membership agreement. No cancellations will be final until the form is submitted. Your membership will be canceled at the end of your current contract term unless you wish to terminate your contract early, per your agreement.

  2. Note that if you have a scheduled renewal payment within this 5-day period, the payment will be processed as scheduled. If you have a contract with an applicable early termination fee, submission of the form authorizes Crossfit 214 to bill your credit card of record to process this fee. All payments are non-refundable.
  3. If you are injured and would like to put your membership on hold instead of cancelling, please email membership@crossfit214.com to discuss options.
